I confirm that I give permission for my child named above to participate in activities run by FreeYouth. This includes occasions, especially in the summer months, where we may take children off-site for activities in the local area. 

All safeguarding guidelines and risk assessments will be taken and met for activities whether on-site or off-site.
 
*Where the child is below the age of 16 years, such processing shall be lawful only if and to the extent that consent is given or authorised by the holder of parental responsibility over the child.
I understand that while present at FreeYouth activities my child will be under the care and supervision of adults with a DBS check and approved by the organising body of Frinton Free Church.  
I understand that while these adults will take all reasonable care of my child, they cannot necessarily be held responsible for any loss, damage or injury suffered by my child during, or as a result of the activities taking place.
 
Whenever medical advice or treatment is needed, the assistance of a GP or A&E Department of a hospital should be sought. The Children’s Act 1989 allows a doctor to provide necessary treatment by doing ‘what is reasonable in all the circumstances of the case for the purpose of safeguarding or promoting the child’s welfare’. I understand that in line with this, every effort will be made to contact me as soon as possible should my child become ill or have an accident and I give permission for my child to be given medical/dental treatment as necessary.
 
I confirm that I give permission for my child to leave the site and return home unaccompanied at the end of the session.
 
During FreeYouth activities, we may wish to take photographs of the events and activities taking place. These could include images of your child. The images may appear in printed publications and/or on our website, YouTube channel and social media sites. To comply with General Data Protection Regulations (GDPR), consent must be granted by a parent/guardian before any images of your child/children are taken and used. The information requested on this form can be completed by a carer, but only those with parental responsibility can sign the consent (N.B. This may not include a foster carer).
